Conditional option on From field not working. Form fails to load and condition option disappears immediately after being added.

Can you share more information about what types of fields you want to create condition for?
Hi, thanks for the follow up. 
The field used to drive the condition is a simple Single Select field and the conditional field is a simple Text field.
It looks like its an issue in the specific form. I just created a new form using just these two fields and the issue does not exist. I kind of looks like its getting picked up in an internal clean up process and removed by that. Strange as it was working fine, now doesn’t. I feel like I encounter this a few times using the app builder… where I can see its doing an internal clean up based on underlying structural changes, but sometimes the logic is not 100% and often I need to refresh the entire page to purge the issue. (Often related to data sources where there have been subsequent changes to the underlying database structure).
I will recreate the entire form later and see if the issue persists in the new form.
I ended up not being able to solve this issue, so recreated the entire form again, and that one is working ok. Some strange bug. 
Apologies for the inconvenience, @DataGecko! We’ll keep a close eye on this to see if we can catch this—or a similar issue with forms—again.
